Completing the Application

How do I fill out the form?

  • The decision will be made based on your testimony about the activities of daily living restrictions and limitations you experience and the medical supporting documents. Both parts are equally important.
  • There are two forms to complete:
  • Canada Pension Plan Disability Benefits Application (“Application”)
  • Medical Report for a Canada Pension Plan Disability Benefit (“Medical Report”)
  • The first form will be filled out solely by you (applicant). Once complete, you send it to Service Canada. You fill out sections 1 and 2 of the second form, and your doctor or nurse practitioner will complete the rest. When the Medical Report is complete, it will be sent by your doctor or nurse practitioner directly to Service Canada.
  • You do not have to wait until your doctor completes the Medical Report. Send the Application as soon as possible since the date of your application my impact when your benefit starts.
  •  You will see that the Application provides you with instructions at the beginning of the form. However, it is a 19-page application, and you may still have questions as your fill it out. Please remember that the Application will include the most crucial information that establishes your eligibility. Where do I find the CPPD benefits application?

  • The forms can be found here. The link will direct you to the Service Canada website. We encourage you to get the form from the official Service Canada website, as it will provide the most recent version.

What do I need to prove that I have a mental or physical disability?

  • Each case is unique and establishing a compelling case will depend on your specific situation. Please note that the following questions are only to guide you with filling out the form.   • Important: If you have more than one medical condition that affects your ability to work, try to answer the following questions for each condition.

What do I need to prove that I have a mental or physical disability?

  • Each case is unique, and how to establish a strong case will depend on your specific situation. Please note that the following questions are only to guide you with filling out the form. If you have specific questions regarding how to answer the questions or what documents to provide, please contact us online or call 416-290-5055.
  • If you have more than one medical condition that affects your ability to work, try to answer the following questions for each condition:
  • What is your medical condition?
  •     My main medical condition is  _______________
  •     My additional medical condition is _______________
  • What have you done to get better?
  •     Did you follow your doctor’s advice?
  •     Did you take the medication or treatment advised by your doctor or nurse?
  •     Did you see the specialists when you were referred to them?
  •     If your answer to the above is no, please provide an explanation
  •  Do you use any aids?
  •     How often do you use them?
  •  Do you take any medications now?
  •     How much do you take them?
  •     How often do you take them?
  • Have you taken any treatments for your medical conditions (s)?
  •     Then you will need documents to support the information you provided. The documents will include, but are not limited to:

o   Record of medical history

o   Letters from doctors

o   Results from medical tests

o   Any other documents that can prove you suffer from a disability/medical condition

I cannot work due to my disability/medical condition(s). Can I get the benefit?

  • In the second part of your application, you need to prove that you lost the ability to work due to your disability or medical conditions. The following questions will help you prepare your answers:

o    What was your job?

o    What did it require you to do?

o   What are your qualifications for the job?

  • your education, expertise, past employment history, language you speak

o    How does your disability / medical conditions(s) affect your ability to work?

  • Think about your:
  • physical abilities
  • behaviour and emotional abilities
  • communication and thinking abilities
  • other daily abilities

o   Think about what you could do before your disability/medical condition(s) got severe and what changed after your disability got severe.

  • In your daily life:
  • Getting ready to work
  • Commuting
  • Performing everyday chores/tasks
  • At your workplace:
  • Your work performance
  • Changes you/employer/co-worker had to make to accommodate you,
  • Changes in your responsibilities/shifts/location
  • Changes in your home

o    Why do you think you may not be able to go back to work?

o    Why do you think that your disability/condition is likely to be long-term or of an indefinite term?

o   Are there other jobs you can do given your age, education, and work history?

How do I send my CPPD benefits application?

  •  You can apply

o   Online by creating My Service Canada Account (MSCA)

o   By paper and mail to Service Canada. You can find the address here

  • Sending your application by Canada Post Express mail will allow you to track the delivery. Before you send, make sure you keep photocopies of everything you submit.

Who fills out the Medical Report for the CPPD benefits application?

  • You (applicant) will answer sections 1 and 2 of the Report. Then the rest will be filled out by a doctor, nurse practitioner, or other health care professionals. You will provide your consent at the end of your Application form so that Service Canada can obtain your information from your doctor to determine your application.
  • Explain to your doctor thoroughly the severity of your conditions and how they affect your ability to work. The focus should be that your disability is severe enough to restrict you from returning to work. You may still be able to perform certain tasks in your daily lives, and your doctor may not think that you are “disabled” based on their definition of disability. In that case, it may be necessary to explain what your job requirements were and why you cannot fulfill them anymore.
  • Terms such as “the patient is unable to perform the duties required for employment” will not automatically pass the eligibility test.  Your doctor must provide reasons as to why they believe you cannot go back to work. In addition, your doctor’s opinion should be supported by their evaluation of your medical condition and personal factors such as age.
  • If your conditions are deteriorating, it should be clearly mentioned in the report. Your doctor may not know that your conditions are deteriorating unless you communicate this to them. If there is any significant change in your condition, make sure you report it to the doctor so that you can receive proper treatment. This will also enable your doctor to reflect the changes on the Medical Report they complete.  

Do I have to pay my doctor for filling out the form?

  •  Service Canada will cover the fee to a certain amount. Your doctor will send the invoice to Service Canada, and the payment will be made. If the fee is more than what is covered by Service Canada, you will need to pay the difference.

Can I get extra benefits if I have children?

  • Yes, your child may be eligible for a Disabled Contributor’s Child’s Benefit. Your child has to be either

o   Under 18 years old, or

o   Under 25 years old and in full-time attendance at a recognized school or university

  • Submit the application form and required documents with your application. You will complete different forms depending on the age of the child. The forms and the instructions can be found here.
  • The payment amount for this benefit changes every year. In 2022, the monthly payment amount for a child is $264.53. You can check the current amount here.
